There are tons of good surf camps, and it all depends on what you are seeking. Many provide not only surfing, but Ecotours, canopy and volcano tours, Spanish classes etc etc.

Personally, I/d go river rafting, canopy and take a trip to the volcano and visits the hots springs if you have time.

As for preparing, make sure you are a good swimmer, suntan lotion, etc. Just about all the surf camps have safety minded teachers since they really do not know the swimming skills of the student, so they watch their new students.

Here are a couple that I have known people who have been quite pleased after attending.

All women surf camp like: http://www.surfdivacostarica.com/ and http://www.witchsrocksurfcamp.com/ , which is somewhat of a boutique surf resort.
